Zusammenfassung Die operative Behandlung thorakoabdomineller Aortenaneurysmen (TAAA) ist eine komplexe Prozedur und erfordert entsprechende Planung. TAAA werden entsprechend der Crawford-Klassifikation (Typen I bis V) eingeteilt. Typ-II-TAAA erfordern den ausgedehntesten Eingriff weisen dementsprechend die höchste postoperative Morbidität Mortalität auf. Perfusionsstrategie Viszeralorgane des Rückenmarks bei operativen Versorgung von zentraler Bedeutung. distale arterielle Perfusion, selektive viszerale Perfusion additiven protektiven Maßnahmen mit Liquordrainage hämodynamischem Management zur möglichst frühzeitigen Etablierung eines pulsatilen Flusses spielen dabei wesentliche Rolle. Techniken extrakorporalen Zirkulation Herz-Lungen-Maschine oder Linksherzbypass ihren Vor- Nachteilen diskutiert. Möglichkeiten viszeralen Organperfusion vorgestellt.
